National Healthcare Decisions Day: Why Planning Matters

National Health Care Decisions Week (April 16-22, 2018) is about helping people of all ages talk about their wishes for end-of-life care, documenting and communicating care preferences to family and health care providers.

Nancy Flowers, LCSW, Community Education Program Manager for Rainbow Hospice and Palliative Care, will share information on documents that you can use to indicate your care preferences, how to choose a health care proxy and how to talk to your family and your doctor about your health care preferences.
